Pragmatic Free Trial Meta

Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It collects and distributes clean trial data, ratings and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses that examine the effect of treatment across trials of various levels of pragmatism.

Background

Pragmatic studies are increasingly recognized as providing real-world evidence to support clinical decision-making. The term "pragmatic" however, is used inconsistently and its definition and measurement need further clarification. Furthermore, pragmatic trials should focus on outcomes that are important to patients, such as quality of life or functional recovery. This is particularly relevant when it comes to trials that involve invasive procedures or those with potential serious adverse events. It is important to understand that a pragmatic trial does not necessarily mean a low quality trial, and in fact there is a growing number of clinical trials (as defined by MEDLINE search, however this is not specific nor sensitive) that use the term 'pragmatic' in their title or abstract. These terms may signal that there is a greater appreciation of pragmatism in abstracts and titles, but it's not clear if this is reflected in content.

Conclusions

In recent years, pragmatic trials are becoming more popular in research as the importance of real-world evidence is increasingly recognized. They are clinical trials randomized which compare real-world treatment options rather than experimental treatments under development, they involve populations of patients that more closely mirror the patients who receive routine care, they employ comparators that are used in routine practice (e.g. existing drugs) and depend on the self-reporting of participants about outcomes.
