Rerolling an item may raise its ilvl in certain instances. However the item won't maintain its ilvl, so rerolling is only useful when an item has rare affixes.
Rerolling an object resets its power base including all upgrades and substats. This is helpful if the power of an upgraded item upgrading is less than what you want or if a player desires different substats on an existing ring. The reroll token can be obtained in a variety of ways like from the mistery box, as a drop from skulled creatures, or as part of the reward for killing the boss.
