The wrong upgrade can ruin the weapon that was previously excellent, and could even render it unusable.
A general rule is that you should only upgrade item an piece of equipment to the level of the highest-level equipment in its category. This means that you are able to upgrade body armors, helms, shields and gloves to the level of their most elite counterparts. Weapons, on the other hand can be upgraded to the level of a sword. This doesn't increase the weapon's power of damage and can reduce its trade value.
You can upgrade an item using flightstones or crests. The amount of each depends on the item level and slot. For example upgrading to a sword will cost more flightstones than an upgrade to bracers or a cloak. Also, you can get discounts on the number of flightstones or crests required to upgrade an item.

Jewels can be incorporated into objects with sockets that match as with other upgrade components. For instance Garnet Pebble (gemstone) and an Adorned Garnet Jewel (jewel) both provide a bonus to strength, but the Garnet Pebble is only able to increase regular hits, while the Garnet Jewel also boosts the player's strength. Jewels can be placed in all tiers of weapon, armor, and accessory slots, as well as some trinkets of rarer or exotic nature which require a slot to be empty.

These requirements are based on the item level and the weapon's requirements are typically higher requirements than armor.
In addition to the required upgrade components, a lot of upgrade recipes have an acceptable materials list. This list includes the other components of an upgrade that can be used to upgrade an item. It also includes a flag that indicates whether or not the item has an assured boost. This is important as the value of an upgrade can vary greatly depending on its 'extras.'
